Privacy Notice

The LASIK Index never asks about your eyes, your prescription or your health. It has no patient accounts and sets no advertising cookies. This notice explains what we do collect and who sees it.

Visitors

Our host, Cloudflare, records standard request data such as IP address and browser type for security and aggregate traffic counts. Maps load from Google only when you click to show a map, at which point Google's privacy policy applies.

Consultation requests

When you ask to be matched with a practice, we collect your ZIP code, the procedure you are interested in, your timeframe, how you would like to pay if you tell us, a phone number or email address, and your name if you give it. With the consent you give on the form, we share those details with the practice you chose, or with up to three practices in the city you chose, so they can contact you. We do not sell them and we do not use them for advertising. We keep a record of the consent wording you agreed to.

Consumer health data (Washington, Nevada and similar state laws)

A request for a LASIK consultation can count as consumer health data because it shows you are looking for a health service. We collect it only from the form above, only with your consent, and we share it only with the practices named or described on that form, with your separate consent to that sharing. You can withdraw consent and ask us to delete your request at any time by writing to hello@lasikindex.com, and we will pass the deletion request to any practice that received it.

Practice forms

When you list or claim a practice, report an error or contact us, we store the details you enter, a one-way hash of your IP address to prevent abuse, and the time. We use them to verify and update records and to reply to you. If you tick the sponsored placement box, we will contact you about it. We do not sell this information. Please do not send us health information.

Practices and doctors

Records are built from public sources listed on the data sources page. Doctors named here are named in their own practice's published materials or the federal NPI registry. To correct or remove a name, use the corrections page.
